I’d like to be able to choose middle name as display name, when appropriate.

There should be a way to choose a person's middle name as their display name in the tree. Eg. My brother is Howard David but has always been known by David.

It's pretty common to put the name that people go by in the "Nickname" field. When you display that field (under Options), it would display in your brother's case as Howard David "David" Smith.
